Please use the following webform to contact us regarding a verified request to: (1) know the categories of personal information collected by Quantum; (2) know the specific pieces of personal information collected by Quantum about you; (3) delete your personal information collected or maintained by Quantum, subject to certain exceptions permitted by law; (4) correct inaccurate information about you; (5) request a copy of your personal information in an easily accessible format and information explaining how that information is used; and (6) appeal a decision already made.
